I understand that participation in camp activities involves physical movement and group participation. I voluntarily allow my child to participate in the camp program and assume responsibility for any risks associated with normal camp activities. I release and hold harmless Theatre for Young America, its staff, instructors, volunteers, and affiliates from any liability, injury, loss, or damage that may occur while my child is participating in camp activities, except in cases of gross negligence. I certify that my child is physically able to participate in camp activities.

I give permission for Theatre for Young America to photograph or record my child during camp activities and to use these images or recordings for promotional or educational purposes, including social media, websites, printed materials, and advertising. No names will be used without additional permission.
